Consent, Intake, and Policy Forms


All new patient forms must be completed in your client portal account before your intake assessment session is scheduled.

A client portal account is established by providing our office with your full legal name, date of birth, and email. If the client is a minor, the portal account is established in the minor’s name with their date of birth, and the parent/guardian’s email. You will then receive a link in your email to set up your client portal account and complete all of the intake forms.

Intake forms include your consent for services, privacy policies, the current fee schedule, insurance information (if applicable), form of payment, background/history, and other person-specific agreements and authorizations.

Once all the forms are complete, a member of our team will reach out to schedule your intake assessment session with a clinician whom our intake team identifies as the best fit for you based on your presenting concerns, availability, and any preferences.  

All forms must be completed in full and received at least three days prior to your appointment to ensure the best use of time during the intake session.

Once your portal account is established, you can use it to update or complete new forms and requests, join telehealth sessions, request medication management appointments, modify contact, insurance, billing, and payment information, upload test results and medical documents, complete outcome measures assigned by your clinician for tracking progress, and send/receive private messages between you and your provider.
