Intake Appointment
If you are interested in receiving services, an intake appointment is required to gather information about you and your concerns before we decide to establish a professional relationship.
The intake assessment session is an opportunity to meet you, obtain more details about your situation and needs, determine if continuing to work together would be a good fit, and, if appropriate, develop a preliminary treatment plan. After this session, we are usually able to provide recommendations for next steps, which often involves scheduling a standing appointment for regular weekly or every other week therapy sessions.
Intakes are managed in different ways when children are involved. The parent/guardian and the clinician will discuss the best way to approach the intake for each situation. Often times, there will be two separate intake sessions - the first with parents/guardians alone so they can talk freely, and the second with the minor either with or without parents for some of all of that session.

Forms
Intake forms are completed online via our secure client portal.
Before scheduling your intake assessment appointment, you will receive an email from Clarity with a unique link to your HIPAA-compliant client portal account where you will find all of the necessary consent and background forms.
All forms must be completed in full and received at least three business days prior to your appointment to proceed with the intake and to ensure the best use of time during the intake session.
Once your portal account is established, we ask that clients use the messaging feature within their portal for all future communications. This maintains client privacy and confidentiality. The client portal is used to update or complete new forms and requests, request medication management appointments, complete screenings, upload documents, among many other features. Insurance and Fees
We are in-network with a select few of the major insurance plans (Currently: Anthem, CareFirst/BCBS, and United/Optum). We are out-of-network with all other insurance plans. If you do not have one of the plans for which we are in network, we are happy to provide superbills that you can submit directly to your insurance for any out of network benefits your plan may reimburse directly to you. For out-of-network services, we do not bill your insurance for services (we accept debit, credit and FSA cards).
You will be asked to upload a copy of the front and back of your insurance card through your client portal account if you wish to use a plan for which we are in-network. Prior to scheduling the initial intake assessment, we will verify your in-network benefits, deductible and copayments. Clarity is required to collect our contracted session rate from you until your deductible is met. Once we are notified by your insurance that your deductible has been met, we will then collect your copayment amount. Co-payments and any unmet out of pocket deductibles are due and collected at the time service. (Note that we can only follow the information on the explanation of benefits provided to us by your insurance company, which we reconcile on an efficient timeline.)
Please contact us for our direct rates. If you are paying out of pocket for services, we will provide a Good Faith Estimate for services in accordance with the No Surprises Act.
All clients are required to maintain an active credit, debit or Health Savings Account (HSA) debit card on file to charge all fees at the time they are incurred. If you wish to change the card on file, if it expires, or if it is declined, please upload a new card immediately. Payment via cash or check can be arranged during the intake process.

Privacy and HIPAA
Your therapeutic relationship is private. Information discussed in sessions will remain completely confidential (with some exceptions for safety and as required by a court order). You have the freedom to choose for yourself what information about your treatment that you wish to share or withhold from others. The Health Insurance Portability and Accountability Act (HIPAA) helps to safeguard your Protected Health Information (PHI).
